Passport Appointment Request

Appointments are for groups of 3 or more applicants only.

To request an appointment for passport application processing, please complete the below information. If you need assistance please contact us at 847-377-3351. Once your appointment has been confirmed an agent will contact you with instructions and requirements for your appointment.

How many applicants 15 years or younger?

If an applicant is 15 or younger both parents must be present. If both parents cannot be present Form DS-3053 must be completed and brought to the appointment along with the application form DS-11.

APPLICATION INFORMATION

Please read the application descriptions and indicate the number of each type of application you will need processed at your appointment:

New Application
Select this option if you have never had a passport before or your passport has been expired 10 years or more. Individuals the age of 15 or younger will be required to fill out a new passport application when renewing their passport. Form DS-11

Total Number of New Applications

Renewal
Select this option if you were previously issued a passport and meet all of the requirements for renewing your passport. Please visit see Passport Renewal information to see if you are eligible for this option. Option is not available for individuals 15 years or younger. Form DS-82

Lost or Stolen Passport
Select this option if your current passport has been lost or stolen. Forms DS-11 and DS-64

Passport Correcton/Change
Select this option if you currently have a valid passport but must update the information that appears on your current passport such as name or gender. Form DS-5504

APPOINTMENT REQUEST

Appointments are Monday through Friday from 8:30 AM to 2:00 PM. We will do our best to accommodate your request, but dates and time are subject to availability. Please allow a minimum of 30 minutes per application.
