BOND REFUND INFORMATION
The Circuit Clerk’s office is required to report to the Internal Revenue Service any bond refunds made to an attorney or attorney firm that is in excess of $600.00 per calendar year. In order for the Circuit Clerk’s Office to comply, we must collect a W-9 form on file from the attorney and/or firm. Please complete the forms below, save them to your computer and upload each document in the spaces provided below.

If you prefer to submit these documents in person you may do so at our office located at 18 N. County Street in Waukegan, Illinois, 60085.
